Smart Card Pin Prompt Missing Pin Field in Google Chrome
Problem
Using Google Chrome for accessing Common Access Card (CAC) enabled website and the PIN prompt does not have field to enter the PIN into.
Google Chrome is NOT the recommended browser for use with DoD issued CAC cards, especially for use with DoD Enterprise Email due to its inability to use S/MIME (Secure/Multipurpose Internet Mail Extensions). More information can be found at

Solution
In Google Chrome add and enable the following Chrome Extensions.
Below is how the referenced add-ins appear in chrome://extensions/ once installed.
